This form should be used by students when submitting an appeal to a decision stemming from the Involuntary Withddrawal Policy made by the CARE Team.  Complete information about involuntary withdrawals, including information about the appeals process, can be found in the Involuntary Withdrawal Policy which is kept in the College Catalog .
